4.本項目需要落實的政策：信用記錄，適用于（本項目），《關于在采購活動中查詢及使用信用記錄有關問題的通知》（財庫【2016】125號）供應商不得被列入失信被執(zhí)行人、重大稅收違法失信主體、政府采購嚴重違法失信行為記錄名單，按照下列規(guī)定執(zhí)行：（1）供應商應在（本項目投標截止時間）前分別通過“信用中國”網(wǎng)站（www.creditchina.gov.cn）、中國政府采購網(wǎng)（www.ccgp.gov.cn）查詢并打印相應的信用記錄（以下簡稱：“供應商提供的查詢結果”），供應商提供的查詢結果應為其通過上述網(wǎng)站獲取的信用信息查詢結果原始頁面的打印件（或截圖）。（2）查詢結果的審查：①由資格審查小組通過上述網(wǎng)站查詢并打印供應商信用記錄（以下簡稱：“資格審查小組的查詢結果”）。②供應商提供的查詢結果與資格審查小組的查詢結果不一致的，以資格審查小組的查詢結果為準。③因上述網(wǎng)站原因導致資格審查小組無法查詢供應商信用記錄的（資格審查小組應將通過上述網(wǎng)站查詢供應商信用記錄時的原始頁面打印后隨采購文件一并存檔），以供應商提供的查詢結果為準。④查詢結果存在供應商應被拒絕參與采購活動相關信息的，其資格審查不合格。
5.供應商的資格要求：
5.1法定條件：符合《中華人民共和國政府采購法》第二十二條第一款規(guī)定的條件。
5.2特定條件：無。
5.3是否接受聯(lián)合體形式的響應磋商：不接受
※（1）根據(jù)上述資格要求，供應商響應文件中應提交的“資格證明文件”相關規(guī)定和資料要求，詳見競爭性磋商須知前附表和磋商文件第三章。
（2）供應商所提供的所有資格及資信證明文件復印件應是最新、清晰、有效，并加蓋供應商公章，原件備查。未達到以上資格標準要求的按無效投標處理。
（3）若供應商提供注明“復印件無效”的證明材料或資料，其資格及資信證明文件紙質(zhì)響應文件正本中應提供原件（資格審查小組將核對資格及資信證明文件紙質(zhì)響應文件正本，未提供原件的證明材料或資料將導致投標無效）。
6.競爭性磋商文件的獲取
6.1競爭性磋商文件提供期限：2022年10月24日 —2022年10月31日（節(jié)假日除外），每天上午9：00～12：00，下午2:30～5:00(北京時間,下同)。
6.2獲取地點及方式：報名后，福建省閩咨造價咨詢有限公司以電子郵件方式或領取紙質(zhì)材料方式獲取競爭性磋商文件?？蓪①F公司所需報名的項目編號、公司名稱、聯(lián)系人、聯(lián)系電話、手機、傳真、郵箱和公司地址一并填寫清楚后傳真或發(fā)電子郵件至本公司報名（傳真或發(fā)電子郵件報名的，須在發(fā)送報名材料后致電我司告知，否則一切后果自行負責），否則投標將被拒絕。
6.3競爭性磋商文件(紙質(zhì)版或電子版)售價均為200元，售后不退。如需郵寄，另加50元人民幣特快專遞費，福建省閩咨造價咨詢有限公司不對郵寄過程中可能發(fā)生的延誤或丟失負責。
7.首次響應文件遞交截止時間及地點：
響應文件應于2022年11月04日下午14:30（北京時間）之前提交到(地址：福州市五四路158號環(huán)球廣場B座13層本項目開標室)，逾期收到的或不符合規(guī)定的響應文件將被拒絕。
